ABSTRACT
OBJECTIVE@#To determine the carrier rate of Fragile X mental retardation 1 gene (FMR1) mutants in women with a history of adverse pregnancy or childbirth, and to provide prenatal diagnosis for the carriers.@*METHODS@#Peripheral blood samples were collected from women with a history of adverse pregnancy or childbirth, and the FMR1 gene cytosine-guanine-guanine repeat number (CGG)n was determined by triple-repeat primer polymerase chain reaction (TP-PCR) combined with capillary electrophoresis. Prenatal diagnosis was provided for the carriers during pregnancy.@*RESULTS@#Among 819 samples, 9 gray zone repeats carriers and 10 premutation carriers were detected, which gave a prevalence of 1 in 91 and 1 in 82, respectively, with a total prevalence of 1 in 43. Prenatal diagnosis was provided during 7 pregnancies for 6 carriers. A female fetus with premutation (n = 30/57) and an affected male fetus with full mutation (n = 336) were detected.@*CONCLUSION@#FMR1 gene testing in women with a history of adverse pregnancy or childbirth can facilitate genetic counseling and reproductive guidance for carriers of gray zone repeats and premutations. Prenatal diagnosis for carriers of premutation can facilitate reduction of the birth of children with fragile X syndrome.

Young women affected by education and career delay the childbearing period,which results in reduced fertility.With the two-child policy completely opening,more and more elderly women have demands for pregnancy and the need of assisted reproductive technology (ART) increases day by day.However,the ART treatment faces with a series of medical and ethical problems.For example,the pregnancy rate of aged women is low but the abortion rate is high;and the incidence of pregnancy complications and fetal chromosomal abnormality increases significantly.In this regard,it can solve these problems through limiting the women's age,choosing the donated eggs,strengthening the counseling for elderly infertile women before ART treatment,paying attention to the psychological problems and applying the genetic screening technology before implantation.

Objective To investigate the correlation between spatial memory and sleep architecture and hippocampal volumes in patients with chronic insomnia disorder.Methods Twenty-two chronic insomnia patients and 17 normal sleepers (controls) were selected to evaluate the subjective insomnia using Pittsburgh Sleep Quality Index (PSQI), Epworth Sleepiness Scale (ESS), Insomnia Severity Index (ISI) and the objective insomnia by polysomnography (PSG).The cognitive function was measured by Montreal Cognitive Assessment (MoCA).Spatial memory and object-memory were measured by Nine-box Maze, and object-recognition memory was detected by picture recognition test.MRI was used to detect hippocampus volumes.Results Compared with controls, a significant reduction in total sleep time (328.3 (310.4, 387.9) min vs 418.0 (375.8, 45.5) min, Z=2.607, P=0.009), sleep efficiency (%) (77.7 (73.1, 84.0) vs 93.0 (87.2, 93.9), Z=3.739,P=0.000), proportion of N3 (%) (5.5 (0.4, 14.4) vs 13.7 (7.7, 18.3), Z=2.664, P=0.008) and proportion of rapid eye movement (REM) (%) (14.4 (10.7, 17.2) vs 17.3 (15.9, 23.3), Z=2.890, P=0.004) was seen in insomnia patients, whereas sleep latency was delayed.The error numbers of spatial working-memory (4.5 (2.0, 7.3) vs 1.0 (0.0, 3.0), Z=3.007, P=0.003) in chronic insomnia patients were more than those in controls.There was no statistically significant difference in object reference memory, spatial reference memory and object recognition in two groups.A significant reduction of the left (2 818.0 (2 534.9, 3 191.8) mm3 vs 3 453.3 (3 081.2, 3 764.4) mm3, Z=3.314, P=0.001), right (2 780.5 (2 451.2, 3 191.8) mm3 vs 3 479.8 (3 024.1, 3 786.7) mm3, Z=3.484,P=0.000) and whole hippocampal volumes (5 561.7 (4 956.6, 6 396.9) mm3 vs 6 898.9 (6 017.1, 7 540.1) mm3, Z=3.455, P=0.001) was seen in chronic insomnia patients compared with controls.The hippocampal volumes were negatively correlated with sleep latency (r=-0.432, P=0.006), but positively correlated with sleep efficiency, proportion of N3 (r=0.323, 0.376;P=0.045, 0.018).There was a negative correlation between the error numbers of spatial working-memory and hippocampal volumes (r=-0.351, P=0.029).The hippocampal volumes were negatively correlated with the duration of disease in chronic insomnia patients (r=-0.734, P<0.01).Conclusion The spatial memory may be associated with decreased proportion of REM and reduced hippocampal volumes in chronic insomnia patients.

Objective To explore the outpatients′experience of caring and provide references for improving their experience of medical care.Methods The phenomenology research method was adopted in this study,and twenty-one outpatients were subject to an in-depth interview to learn their outpatient experiences.Results Ten themes of experience with caring were extracted as follows:passionate and patient,careful and considerate,respect for others,offers to help,encouraging and comforting,apologies and courteously,humor,truthful considerations,conscientious and responsible,and a caring environment setup.Five themes of experience without caring were extracted as follows:impatient communication, careless seeing of outpatients,delayed service,passive reminders,and facilities without enough details. Conclusions The outpatients may have abundant experiences of caring at the outpatient clinic,also lack of caring sometimes as well.Outpatient managers should keep an eye on outpatients′needs and satisfaction of caring from time to time,to improve the quality of outpatient service.

Objective To analyze CMV DNA stability of 30 EDTA plasma samples in the order of magnitude between 300 and 100 000 copies/mL over a 21 day period. Methods Thirty plasma samples were grouped into three categories according to the CMV DNA loads , including low CMV DNA contents , intermediate CMV DNA loads and high CMV DNA loads. Ten milliliters of whole blood was freshly collected from each patient. Plasma samples without hemolysis were divided into 1-ml aliquots. One aliquot was processed immediately (Day 0) for baseline PCR assays. The remaining aliquots were then processed after one , two, three, seven, 14 or 21 day of storage at 4℃. Results There was no significant difference between the mean of the difference time point in viral loads following storage at 4 ℃ by paired-samples t test, including Day 1 compared to Day 0 (t = 1.654, P =0.109), Day 2 compared to Day 0 (t = 1.487, P = 0.148), Day 3 compared to Day 0 (t = 1.609, P = 0.118), Day 7 compared to Day 0 (t=0.831, P=0.413), Day 14 compared to Day 0 (t=1.721, P=0.096), and Day 21 compared to Day 0 (t=0.244, P=0.810). Conclusion The concentration of CMV DNA in all samples stored at 4 ℃ for 21 days did not differ significantly from the baseline viral load ,and it was not observed the trend in continued degradation in different time point (Day 1, 2, 3, 7 and 14).

From members in this local district from January 2011 to December 2013,a total of 150 patients with malignant cancer and chronic disease patients of lost of functions and their carers were recruited.A self-tailored interview was conducted via face-to-face communications.And according to 112 (74.7％) carers,as compared to hospitalization,the patients were more likely willing to be looked after at home.The 95 (63.3 ％) carers had a lack of professional caring knowledge and their caring techniques were insufficient.And 97 (64.7％) carers were willing receive professional training in nursing and protecting dying patients.In-house terminal care is indeed required by the patients.However its service is best supported by the following aspects.The supports from local medical services and resources; professional training organized by a local social (neighborhood) committee and providing the guidance of in-house caring techniques and information of the relevant disease management.Building up a mutual terminal care group and finalizing the caring standards as soon as possible are essentials.

OBJECTIVE@#To evaluate the abilities speech perception of post-lingual cochlear implant patients by Mandarin Hearing In Noise Test(MHINT), get the performance-intensity function (P-I function).@*METHOD@#The material was MHINT adults' version, including 12 lists, 20 sentences per list. Twenty-two cochlear implant patients were the subjects, age from 14 to 56. Three adaptive rules were used during the test. Firstly Rule 3 would be done. If subject could finish the test, then Rule 2 and Rule 1 would be used. Got the Reception threshold of sentences and speech recognition score of subjects, then got performance-intensity function (PI function).@*RESULT@#Among those 22 subjects, 5 of them could be tested in all three rules, 3 could be tested in Rule 2 and Rule 3, 7 of them could be tested with Rule 3, and 7 of the CI users could not be tested even with Rule 3. All of the PI functions for CI users are shifted 10 dB or more above the normal PI functions. Most CI users thought that MHINT was hard for them.@*CONCLUSION@#When use new adaptive scoring rules for MHINT, most CI users could be tested with the modified HINT scoring rules, although three subjects could not be tested. Compared with normal hearing subjects, all CI users required positive S/N ratios to perform the HINT measures.

Objective To investigate the protective mechanism of octahedral montone in rats with acute pancreatitis.Methods Seventy-two SD rats were randomly divided into a sham-operation ( SO) group, a severe acute pancreatitis (SAP) group and a treatment with octahedral montone group.Retrograde pancreatic ductal injection of 5% cholate sodium in rats was used to establish SAP models.Sham operation was done with intraperitoneal injection of normal saline.In the treated group octahedral montone was given through enema half hour before inducing SAP model.Then, we evaluate the pancreatic injury and detect the level of TNF-alpha, diamine oxidase ( DAO) and endotoxin.Western blot and RT-PCR were used to determine the expressions of the tight junction protein occludin in the endothelial cells of intestinal mucosa at the time of hour 3,6, 12 after operation.Results ( 1 ) The pathological scores of pancreatitis were significantly higher in the SAP group than those in the treatment group and SO group (P < 0.05 ).(2) Compared with the SO group, the level of TNF-alpha in the SAP group and the treatment group was much higher ( P < 0.05 ) , but the level in the treatment group was lower than that in the SAP group ( P < 0.05 ).(3) The serum concentration of DAO and endotoxin was significantly increased in the SAP group, and the concentration in treatment group was higher than that in the SO group (P <0.01) , but lower than that in the SAP group ( P <0.01 ).The occludin protein and mRNA expression in the SAP group was the lowest and the expression in the treatment group was higher than that in the SAP group (P <0.01) ,but lower than that in the SO group (P < 0.01).Conclusions Octahedral montone can improve the colonic barrier function, reduce the endotoxemia, and ameliorate the inflammation during acute pancreatitis.

Objective To investigate the role of octabedral montone on ileum NF-κB activation in rats with acute necrotizing pancreatitis (ANP).Methods 120 SD rats were randomly divided into sham-operation group(A),ANP group(B),mosapride treatment group(C),montone treatment group(D),montone and mosapride treatment group (E).Retrograde pancreatic duct injection of sodium taurocholate was used to establish ANP models,octahedral montone and mosapride were given by intragastric feeding 30 min before ANP model induction,group A was injected NS after opening the abdomen.Pancreatic tissue injury was evaluated;the serum level of TNF-a and IL-1 was determined;the ultrastructural changes of the ileum was observed.The expression of NF-κB in the ileum mucosa was detemined by immunohistochemistry.The expression of IκBa protein in the ileum tissue was determined by Western blotting.Results Compared with group A,pancreatic tissue injury was more severe in group B,and serum level of TNF-a and IL-1 was much higher in group B [(36.79±1.14) vs (9.00±0.01 )pg/ml,(25.17±1.93) vs (3.45±0.11)ng/ml,P＜0.05];the height of the villi was significantly lower[(0.25±0.22 )×10~(-6) mvs(1.50±0.33 )×10~(-6) m,P＜0.05)],the index of columnar cell was significantly lower[(2.40±1.65)×10~(-6)m vs (13.5±4.28)×10~6/m,P＜0.05)];the expressions of NF-κB was significantly eleveted(6.92±1.54vs 1.28±0.29,P＜0.05),the expressions of IκBa protein was significantly decreased(3.30±1.99 vs 24.32±1.93).There was no significant difference between group D and C.The serum level of TNF-a and IL-1[(30.57±0.39)pg/ml and(13.45±1.26)ng/ml],the height of the villi and the index of columnar cell[(1.05±0.28)×10~(-6)m and(10.10±2.50)×10~6/m]and the expressions of NF-κB(4.3240±1.5657)and IκBa(15.987±1.262)was statistically significantly different compared with those of the ANP group(P＜0.05).Conclusions Octahedral montone and mosapride could decrease the NF-κB activation and ameliorate the inflammation of ileum and pancreas during the course of ANP.
ABSTRACT
This study investigated the influence of silencing TRAF6 with shRNA on lipopolysaccharide (LPS)/toll-like receptor (TLR)-4 signaling pathway in vitro. Four plasmids (pGCsi-TRAF6-shRNA1, 2, 3, 4) containing different shRNA sequences were designed and synthesized. The proliferation of RAW264.7 cells after transfected with these plasmids was measured by MTT assay. Inflammatory cellular models were established by LPS stimulation. Levels of TNF-alpha, IL-1beta and TGF-beta1 in the supernatants, mRNA expressions of TRAF6, IL-6 and COX-2, protein expression of TRAF6 and translocation of NF-kappaB were assayed by ELISA, real-time quantitative PCR and Western blotting, respectively. The results showed that the TRAF6 gene knockdown by RNAi hardly inhibited the proliferation of RAW264.7 cells within 72 h. The mRNA and protein expression of TRAF6 was lower in the TRAF6-shRNA1, 2 groups than in the TRAF6-shRNA3, 4 groups. Therefore, pGCsi-TRAF6-shRNA1, 2 were selected for the subsequent experiments. Our results still showed that pGCsi-TRAF6-shRNA1, 2 could significantly reduce the production of pro-inflammatory cytokines and mediators including TNF-alpha, IL-1beta, IL-6 and COX-2, and inhibit NF-kappaB nuclear translocation. Moreover, pGCsi-TRAF6-shRNA1, 2 could suppress the release of TGF-beta1 at the protein level. It was concluded that the recombinant plasmid pTRAF6-shRNA can, to some extent, inhibit inflammatory response stimulated by LPS at the initial phase. TRAF6 may become the potential therapeutic target of many inflammation-related diseases.

In order to enhance the utilization efficiency, reduce the environmental pollution of traditional chemical treatment and the agriculture waste incineration; we studied the ligninase production by Coprinus comatus, Aspergillus niger and Trichoderma reesei through the plate screening. The results showed that C. comatus mixed culture with T. reesei have a good compatibility and higher yields of Laccase. On the basis of this pre-experiment, we studied the optimal conditions of mixed culture for enzyme production. Under the optimal conditions: the inoculation proportion of C. comatus and T. reesei (5:2), the interval of time (12 h), the temperature 260C, the shake rotation speed 150 r/min, fermented for 3 days, the Laccase activity reached 3267.1 U/mL, increased by 106% contrasted with single culture of C. commatus.

Objective Base on the neighborhood activation model(NAM), to develop the Mandarin monosyllable lexical neighborhood test(M-LNT-monosyllable), which is expected to be useful for evaluating speech perception performance in children with hearing loss. Methods Test items were based on the lexical characteristics of word frequency and neighborhood density which addressed in the neighborhood activation model (NAM). M-LNT -monosyllable consisted of two parts: Lexically "easy" words with high word frequency, which were low phonemically similar to the target word and lexically "hard" words with low word frequency, which were high phonemically similar to the target word. 34 children of 3～5 year old with normal hearing were choosed as subjects to verify easy word and hard word lists. Results 1 979 words for children contained 487 easy words and 419 hard words. Three easy word lists and three hard word lists were developed to estimate the performance of word recognition among normal- hearing children. There were no differences among scores of three easy words lists(P＞0.05), and no difference among scores of three hard words lists(P＞0.05). But there were significant differences between scores of easy and hard words lists(P＜0.01). Conclusion The development of the lexicon was affected by the lexical characteristics. Normal-hearing children with some lexical techniques were affected by the lexical characteristics when they recognized the spoken words, but children with less lexical technique didn't show the same result because the recognition was processed on the phonetic level.

@#ObjectiveTo compare the effect and prognosis of two kinds of internal fixation (improved Giebel blade plate and traditional straddle nail) after high tibial osteotomy (HTO) on osteoarthritis of knee with genu varus deformity. Methods37 knees of 32 cases were treated with straddle nail (25 knees) or Giebel blade plate (12 knees). All the cases were followed up for 6～28 months. ResultsThe clinical bone healing time of osteotomy was 8～12 weeks. There was no significant differences between 2 groups in the increased score in HSS Standard and in the clinical bone healing time. ConclusionBoth internal fixation with improved Giebel blade plate and traditional straddle nail get similarly satisfactory prognosis, while the former shows more advantages to allow early functional exercises.

AIM: To investigate the multidrug resistance(MDR),reversal activity of 2-[4-(2-pyridin-2-yl-vinyl) henyl]-4,5-bis-(4-N,N-diethylaminophenyl)-1(H)-imidazole(FG020318) in a retinoblastoma subline SO-Rb50/VCR,which is resistant to vincristine.METHODS: The procedure of stepwise increase in drug concentrations was used to obtain SO-Rb50/VCR,which was resistant to 200 ?g/L vincristine. The chemosensitivity of this drug resistant cell line with and without FG020318 or cyclosporine A(CSA) were detected by MTT assay and the function of p-glycoprotein(P-gp) was examined by rhodamine 123 accumulation detected with flow cytometry(FCM).RESULTS: FG020318(2.5 ?mol/L) significantly reduced IC50 and increased the rhodamine accumulation in a concentration-dependent manner. It was much stronger than the positive control CSA in reversal of MDR.CONCLUSION: A new tumor MDR modulator FG020318 partly reverses MDR in SO-Rb50/VCR.It may be a promising new drug to tackling MDR.

Objective To evaluate the open-set auditory speech perception abilities, to characterize the auditory speech development of children with cochlear implants.Methods 27 congenitally deafened children with cochlear implants were evaluated with Mandarin monosyllable and disyllable lexical neighborhood test(M-LNT-Monosyllable & M-LNT-Disyllable) and Mandarin hearing in noise test for children(MHINT-C) according to the order from M-MLNT easy test,M-MLNT hard test, M-LNT easy test ,M-LNT hard test, MHINT-C quiet condition, MHINT-C noise front condition, MHINT-C noise non-implant side condition to MHINT-C noise implant side condition in sound field.Results 27 CI children have completed M-LNT/M-MLNT, 9 of them could be tested in quiet condition with MHINT, and 7 could be tested in noise with MHINT.
